Michael Guidry, MS, CNS, LDN = $120/ hour.
INITIAL VISIT
Includes health history, evaluation, clinical exam, ordering lab tests, and any initial diet and
supplement recommendations. Time: 60 – 90 minutes. Please download, print and fill-in the
Intake Form. Bring any recent lab tests. [CPT code = 97802]
SECOND VISIT
Includes nutritional evaluation of laboratory testing, diet counseling, supplement program design.
You will receive an explanation and copy of test results; but they will NOT be mailed or faxed in
advance to you due to federal CLIA regulations. Time: 60 - 90 minutes. [CPT code = 97803]
6-MONTH REVIEW & ANNUAL VISITS
Includes review visit to evaluate your progress and adjust your program. Time: 30-60 minutes.
Recommended at 6 months and once yearly thereafter. [CPT code = 97803]
GROUP VISITS
Family or couple visits for dietary counseling usually take 1 - 2 hours. [CPT code = 97804]
SHORT INTRODUCTORY CONSULT - Optional
A brief nutritional evaluation of your Blood Chemistry, CBC, and Lipids from your physician. Time:
15-20 minutes. Fee: $37.50 - $50.00.
PHONE & EMAIL CONSULTS
Short phone calls or emails (5-10 minutes) are free, and will be returned when time allows.
Longer scheduled phone consultations and any email program reports are billed at the hourly rate,
and a credit card is required at the time of the consult.
CANCELATIONS
Last-minute cancelations and missed appointments are extremely disruptive to our schedules.
Therefore, a fee of $25 will be charged, unless notice is given at least 24 hours in advance. Please
try to give notice before noon of the previous business day (M - F)
LABORATORY TEST FEES
Hair tests are $85. Lab kits for specialty labs are $20. Any other lab fees or co-pays are paid
directly to the laboratories, who will bill insurance directly. See Evaluation & Testing.
COMPUTERIZED DIET ANALYSES
(1) Food Pharmacy Software for food choice recommendations. Fee = $25. [CPT code = 99090]
(2) Diet Sleuth Software evaluation for dietary macronutrients. Fee = $25 / day evaluated.
REPORTS
A short Letter of Medical Necessity with diagnosis and treatment codes is $15 to $25. A longer
formal written report for schools or doctors is $75 / page (single space). [CPT code = 99080]
PAYMENT
Payment is due with each office visit, unless other arrangements are made in advance. Fees are
payable by cash, check, Visa, MasterCard, or Discover. Lab tests & supplements are extra. Any
unpaid balance will be charged interest at 10% per annum. A fee of $15 will be charged for
bounced checks.
REFUNDS
Supplements may only be returned if they are factory sealed and un-expired, and we are still
carrying that product. Lab kits may only be returned if they are intact and un-expired, and we are
still carrying that kit. Credit Memos will be issued. Call first to confirm.
SUPERBILLS
A “Superbill” is supplied with diagnostic codes (ICD-9) and procedural codes (CPT) for office visits,
lab test kits, reports and supplements.
INSURANCE
INSURANCE PARTICIPATION & FILING
Our Clinic does NOT participate directly with any insurance companies, and does NOT file claims.
We will give you a "Superbill", and you are responsible to obtain a claim form from your insurance
company and file your claims.
INSURANCE COVERAGE – LAB TESTING
Most lab tests are billed directly by the lab to your insurance company. HMO’s cover Lab Corp or
Quest Labs. Only PPO’s and Medicare cover specialty nutrition labs; and there may be a modest
co-pay. A small fee is charged by the Clinic for specialty lab kits ($20).
INSURANCE COVERAGE – VIRGINIA
Virginia has a title law regulating who may call themselves a “nutritionist”. But Virginia law does
not yet license nutritionists or dietitians nor mandate insurance coverage for nutritional services.
But diabetes is usually covered. You might also obtain coverage by a letter of referral from your
physician.
INSURANCE COVERAGE – MARYLAND
Maryland licenses nutritionists, including Dr. Power. If you have a Maryland insurance policy,
Maryland law mandates insurance coverage of nutritional services for: cancer, diabetes,
malnutrition, stroke, heart and kidney diseases, and obesity in conjunction with any of these 6
conditions -- up to 6 visits per year.
INSURANCE COVERAGE – WASH DC
Washington DC licenses nutritionists. Insurance coverage of nutrition visits will depend on
Washington DC law. Check with your insurance company.
INSURANCE COVERAGE - FEDERAL (Gov't)
Insurance policies for federal employees defer to state laws on insurance coverage - meaning the
state in which you live.
INSURANCE COVERAGE - TRI CARE (Military)
Tri Care does not have a category for "Nutritionists", and therefore does not cover nutrition
services.
INSURANCE COVERAGE - MEDICARE
Medicare law mandates insurance coverage of office visits with Licensed or Certified Nutrition
Professionals. Dr. Power applied for Medicare Providership in mid-February 2009, and is still
awaiting a response, which takes many months.
INSURANCE COVERAGE – MEDICAID
Virginia Medicaid does not have a category for “Nutritionists”, and therefore does not cover
nutrition services. We do not participate with Maryland Medicaid.
