CONSULT FEES

Laura Power, MS, PhD, LDN  =  $150 / hour.

Starting December 1, 2011, fees will be $160 / hour to accommodate increasing business

expenses.

FIRST VISIT

Includes health history, evaluation, clinical exam, ordering lab tests, and some initial

recommendations.   Time: 60 - 120 minutes, average = 1.5 hours.   Download, print and fill-in

the Intake Form.  Bring recent lab test results.

SECOND VISIT

Includes nutritional evaluation of lab testing, diet counseling, and supplement program.  You will

receive a copy of test results at this time.  Time: 60 - 120 minutes.

FOLLOW UP VISITS

Includes review visit to evaluate your progress and adjust your program.  Time: 30-60 minutes.

Recommended at 6 months and once every year or two thereafter.

PHONE & EMAIL CONSULTS

Short phone calls or emails (5-10 minutes) are free, and will be returned when time allows.

Longer scheduled phone consults and email programs are billed at $150 / hour, and a credit card

is required at the time of consult.

CANCELATIONS

A fee of $25 will be charged, unless notice is given at least 24 hours in advance.

LABORATORY TEST FEES

Hair tests are $90.  Kits for specialty labs are $20.  Other lab fees and co-pays are paid directly

to the laboratories, who will bill insurance directly.  See Evaluation & Testing.

COMPUTERIZED DIET ANALYSES

(1)  Food Pharmacy Software.  Fee = $20.  [CPT code = 99090]

(2)  Diet Sleuth Software.  Fee  = $25 / each day evaluated.

REPORTS

Formal written reports for schools or doctors are  $75 / page (single space).

PAYMENT

Payment is due with each office visit.  Fees are payable by cash, check, Visa, MasterCard, or

Discover.  Lab tests & supplements are extra.

REFUNDS

Supplements may be returned if they are factory sealed and un-expired, and we are still carrying

that product.  Lab kits may be returned if they are intact and un-expired, and we are still

carrying that kit.  Credit Memos will be issued.  Call first to confirm.

SUPERBILLS

A “Superbill” is supplied with diagnostic codes (ICD-9) and procedural codes (CPT) for office

visits, lab test kits, reports and supplements.

INSURANCE

OFFICE VISITS

Your insurance company will pay for office visits IF your policy covers nutrition services.  We are

not an "In Network Provider" and do not file claims.  But you can download a Claim Form from

your insurance company's website, attach our Superbill, and file your claims.

SUPPLEMENTS

Nutritional supplements are not covered by insurance, but may be covered by Health Savings

Plans or Flex Plans.

INSURANCE COVERAGE – LAB TESTING

Most lab tests are billed directly by the lab to your insurance company.  HMO’s cover Lab Corp or

Quest Labs.  Only PPO’s and Medicare cover specialty nutrition labs; and there may be a modest

co-pay.  A small fee is charged by the Clinic for specialty lab kits ($20).

INSURANCE COVERAGE – VIRGINIA

Your insurance company will pay for office visits IF your policy covers nutrition services.  PPO's

usually cover Diabetes.  HMO's may sometimes cover diabetes services at a reduced rate.   A

1999 Law mandates insurance coverage of biologically-based mental illness, specifically including

Autism and ADHD.  This law covers children and adults.  It applies to all insurance policies

including PPO's and HMO's.  We are currently investigating it's relavance to nutritionists.  To see

INSURANCE COVERAGE – WASH DC

Your insurance company will pay for office visits IF your policy covers nutrition services.  PPO's

usually cover Diabetes.  HMO's will sometimes cover diabetes services at a reduced rate.

INSURANCE COVERAGE - FEDERAL (Gov't)

This means insurance policies for federal government  employees.  Your insurance company will

pay for office visits IF your policy covers nutrition services.  PPO's usually cover Diabetes.   HMO's

will sometimes cover diabetes services at a reduced rate.

INSURANCE COVERAGE - TRI CARE  (Military)

TriCare Standard finally has a category for "Nutritionists".  However, covered services are limited

to diabetes and renal disease (kidneys).  Dr. Power is applying to become a Certified Non-

Network Provider.  This requires specific educational and licensing requirements that Dr. Power

meets.

INSURANCE COVERAGE - MEDICARE

We do not participate with Medicare.

INSURANCE COVERAGE – MEDICAID

Virginia Medicaid does not have a category for “Nutritionists”, and therefore does not cover

nutrition services.  We do not participate with Maryland Medicaid.
